Webinar Description: Vaginal radical trachelectomy (VTR) has revolutionized fertility-sparing surgery in cases of early-stage cervical cancer, offering hope to women wishing to preserve reproductive potential without compromising oncologic safety. First pioneered by Dargent in 1994, this minmal invasive surgical technique enables removal of the cervix and parameters while maintaining the uterus, thus enabling the patient to achieve future pregnancies.

This webinar will explore the surgical technique of VRT, highlighting operative steps, including uterine artery preservation, parametrial resection, and the critical role of isthmic cerclage. We will discuss patient selection criteria, identifying optimal candidates and situations where alternative approaches are indicated.

Participants will gain valuable knowledge into oncologic outcomes, recurrence risks, and how key tumor characteristics—such as tumor size, lymphovascular space invasion, and deep stromal invasion—impact prognosis. The session will also explore the fertility and obstetric outcomes following VRT, addressing pregnancy rates, common complications such as cervical insufficiency and preterm birth, and strategies to optimize reproductive success, including cerclage placement and surveillance protocols during pregnancy.

Emerging innovations, such as sentinel lymph node mapping and neoadjuvant chemotherapy for larger tumors, will also be reviewed, alongside future directions aimed at improving both cancer control and fertility preservation.

By the end of this session, attendees will be equipped with practical knowledge to counsel and manage reproductive-aged women considering VRT, balancing oncologic safety with the desire for future childbearing.
